From the wallet qt it is easy. Go to the Receive tab, Label the new address if needed, then click the request payment button.
From where to get that form. I havent create my litcoin wallet & I dont know where to create it.

The image is from a Windows wallet sourced from https://litecoin.org/ .
If you are on a Windows desktop you would need to download the corresponding version according to your hardware specifications (Windows 32 bit version for 32 bit processor, etc.) If using other Operating system or hardware devices download those versions that fit your device.
The wallet graphic user interface may look slightly different but the functionality should be similar.
